Course Evaluation
Students are required to complete a Course Evaluation form at the end of each course and before the final grade for the course is received. No grades are issued unless the evaluation is received. Course Evaluations are to be sent by snail mail to the Administration Office. You may download and print out the following:

A rating of 1 indicates an excellent score. A rating of 3 indicates an average score, 6 a very poor score.
